Introduction
The goal of this program is to support people with diabetes or pre-diabetes in developing the knowledge and skills needed to become self-managers of their diabetes and to improve their quality of life.
We offer free individual counselling and group education with translation services available as needed in most languages.
We help our clients to develop personalized goals to improve their health and prevent diabetes complications.
Family members and/or caregivers are encouraged to attend appointments and groups.
- For people with Pre-diabetes, we offer 1-2 individual appointments.
- For people with Diabetes, we can offer ongoing follow up (typically every 3-6 months). If you decide to leave our program, you are welcome to return at any time in the future.

Clinicians
Diabetes Educators (Registered Nurses and Registered Dietitians) see clients on an ongoing basis to provide education and support with diabetes management, and complication prevention.
