Case Management/Advocacy

Case management aims to help people achieve more stability and security by connecting them to services and resources in their community to address social determinants of health and basic needs (i.e., financial security, food security, housing issues). Case management at Taddle Creek is short-term and predominantly focused on immediate needs and care coordination. This service includes a variety of things, including but not limited to:

  • Assistance with applications (Ontario Works, Ontario Disability Support Program, Ontario Drug Benefit Program, Rent-Geared-to-Income Housing, The Access Point, etc.).
  • Connection to food security programs
  • Connection to employment agencies
  • Exploring opportunities to address social isolation
  • Referral to community agencies and organizations to support unmet needs

Taddle Creek does not offer long-term case management services; however, your social worker can help refer you to agencies that may offer longer-term case management.
