What is the “BETTER For All” program?

BETTER For All is a free program designed to support patients ages 40-69 in reducing their risk of chronic diseases including Cancer, Heart Disease, Type Two Diabetes Mellitus and Osteoporosis. The program connects participants with Prevention Practitioners to collaborate and set personalized health goals. Participants are then provided with the opportunity to be matched with a Peer Health Coach for support and guidance in achieving those goals. Whether you want to be more active, eat healthier, and/or meet your cancer screening goals, BETTER For All can help make it happen.

Who is involved in running this program?

  • Canadian Cancer Society (CCS): Leads the overall program
  • Access Alliance: Supports implementation
  • Women’s College Hospital (WCH): Leads the evaluation and research

Who is eligible to participate?

  • Internal patients only (patients registered with a Taddle Creek FHT Primary Care Provider)
  • Adults 40-69 years of age
  • People who want guidance on reducing their risk of developing chronic diseases
  • Patients with and without access to private dietitian coverage are eligible.
  • Please note this program is not appropriate for patients with an Eating Disorder history. Please see the 1:1 counselling services provided within the Nutrition Program for more details on how to receive Eating Disorder support.

How do I sign up?

Eligible patients can call 416-260-1315 ext 0 and speak with one of our administrative assistants to register. Spots are limited and are booked on a first-come, first-serve basis. Sessions take place every other Friday afternoon.

What happens after I sign up?

  1. The admin team will check your eligibility.
  2. You’ll be booked for the next available session.
  3. You’ll receive a comprehensive health survey to complete at least 24 hours before your appointment. If the survey isn’t submitted 24 hours in advance, your appointment will need to be rescheduled.
  4. You will visit with a Prevention Practitioner to identify health and screening goals, who will also connect you with optional virtual support from a Peer Health Coach for up to 6 months.
  5. You will be asked if you are interested in participating in the evaluative research program through Women’s College Hospital. Participation is optional and provides two $10 gift cards upon completion of two surveys.

Will I continue to see the Dietitian after the visit?

This program includes a single session only. If you’d like to follow up, you may request to book one Single Session visit separately. You are not added to the Dietitian’s ongoing caseload unless you receive a formal referral for 1:1 nutrition counselling. If you wish to receive ongoing support from the Dietitian, please reach out to your Primary Care Provider to determine if you are eligible for a formal referral to the Nutrition program.

What if I miss my appointment or forget to submit the survey?

The survey helps guide your visit and ensures the session is personalized to your needs. It must be completed at least 24 hours before your appointment—otherwise, your appointment will need to be rescheduled. If you miss your appointment, you can feel free to book another spot by calling 416-260-1315 ext 0 to speak with one of our administrative assistants to re-register. Please note that appointments are provided on a first come, first serve basis.

What is the difference between the Single Session Clinic and BETTER For All appointments?

The Single Session Clinic offers a one-time, 60 minute appointment for patients who have a nutrition related issue or concern and would like to have one consultation with our Registered Dietitian. Most common topics covered include: meal suggestions, supplement reviews, accessing food banks/community meals, and ensuring nutritional adequacy in the presence of dietary restrictions. Single Session appointments are available every other Wednesday at 11:00am on a first come, first serve basis.

The BETTER For All visit is targeted specifically to patients ages 40-69 who are looking to prevent certain cancers (breast, prostate, colorectal, cervical and lung cancers) and chronic diseases (Heart Disease, Type Two Diabetes Mellitus and Osteoporosis). The program connects participants with Prevention Practitioners to collaborate and set personalized health goals. Participants are then provided the opportunity to be matched with a Peer Health Coach for support and guidance in achieving those goals. You will be asked if you are interested in participating in the evaluative research program through Women’s College Hospital. Participation is optional and provides two $10 gift cards upon completion of two surveys. Whether you want to be more active, eat healthier, and/or meet your cancer screening goals, BETTER For All can help make it happen. BETTER For All appointments are available every other Friday at 2pm on a first come, first serve basis. Call 416-260-1315 ext 0 and speak with one of our administrative assistants to register.

What is the role of the Peer Health Coach?
